Obtaining a Letter of Recommendation from Mr. Fredericks

You do not need to have been an A student to get a letter of recommendation. However, you should have shown true effort by coming to flex tutorials and participating in class. In addition, you should have shown respect to all members of the class and myself.

To obtain a letter of recommendation, you must provide the following items. Do not provide any additional items (resume', etc...).
  1. Provide addressed, stamped envelopes for the schools. (I do not write general letters, or letters in advance.)
  2. Any forms that the school needs (i.e. The Common Application). Please make photocopies if you are applying to multiple schools.
  3. A completed copy of the following form. Print this page and complete it.
